TERMS & CONDITIONS FOR HEALTHCARE PROVIDERS

The following terms and conditions apply to healthcare professionals referring patients to, or receiving information about patient participation in, any patient support program (Program) sponsored by Biogen Australia Pty Ltd (Biogen) and run by Partizan Worldwide Pty Ltd (Partizan).


You acknowledge and agree that:



  1. You are registered to practice as a healthcare professional in Australia or New Zealand.

  2. The Program does not constitute medical advice and you remain responsible for any medical management and treatment decisions regarding the patient.

  3. Your registration on the Program system will continue for the duration of the Program or until you opt out.

  4. You may opt out at any time by calling 1800 937 497 (NZ 0800 897 227) or writing to the Zephyr Program at Partizan, Reply Paid 86597, North Sydney NSW 2059 Australia (no stamp required). You will otherwise remain registered in the Program system, even if none of your patients are enrolled in the Program.

  5. Services provided under each Program may vary depending on:

    1. Program information provided to you;

    2. Any available service preferences chosen by you for your patients; and

    3. The needs of the relevant patient, depending on the Biogen treatment that they have been prescribed.



  6. Biogen reserves the right to alter or cancel any services under the Program, or the Program itself, at any time.

  7. You consent to Partizan and/or Biogen collecting and using your personal information including contact details for the purpose of enabling your interaction with the Program, as a treating healthcare professional. Partizan and Biogen abide by privacy laws applicable in Australia and New Zealand. ZEPHYR PROGRAM SERVICES


The Zephyr Program is administered by Partizan on behalf of Biogen Australia. A support program for patients prescribed a Biogen therapy for spinal muscular atrophy (SMA), the program consists of a number of different components. The exact services your linked patients are eligible to receive will be based on the patient’s individual needs and their linked HCP preferences.


  1. Telephone based disease and treatment education, and health and wellbeing support delivered by trained nurses and support consultants. The Zephyr team will also seek to understand their wellbeing needs and provide or direct the patient to further information and services that may assist with their broader wellbeing needs. Please note the Zephyr team may provide information about third party service providers (such as Occupational Therapists, Dietitians, or Physiotherapists) located in the patient's area or region. They do not endorse or make any representation about the appropriateness of such services for the individual patient or provide funding for consultations with such service providers.

  2. Disease and treatment education, and health and wellbeing support delivered in the home by trained nurses for patients and their parents/carers. This entails a prearranged visit by a Zephyr nurse who will have a discussion with the patient and their parents/carers about their individual circumstances and health needs, provide education to the patient and their parents/carers on their prescribed Biogen treatment, and wellbeing support depending on specific patient needs. This service is only available if the linked healthcare professionals have nominated this option in the preferences section of the Zephyr HCP Portal.
  3. Telephone based disease and treatment education, and health and wellbeing support delivered by trained nurses and support consultants. The Zephyr team will also seek to understand their wellbeing needs and provide or direct the patient to further information and services that may assist with their broader wellbeing needs. Please note the Zephyr team may provide information about third party service providers (such as Occupational Therapists, Dietitians, or Physiotherapists) located in the patient's area or region. They do not endorse or make any representation about the appropriateness of such services for the individual patient or provide funding for consultations with such service providers.
  4. The provision of support and educational materials specific to spinal muscular atrophy (SMA), their prescribed Biogen medication, and their individual wellbeing and health needs. Depending upon their communication preferences the patient may be provided with materials to support their Zephyr program discussions.
  5. Access to the Zephyr Website. All people enrolled in the Zephyr Program receive access to the website with information specific to SMA, their prescribed Biogen medication, and their individual wellbeing and health needs. The Zephyr website also includes access to functionality to set treatment and other reminders, request materials relevant to their prescribed treatment, and request a Zephyr support call.


As part of the Zephyr Program, registered HCPs have access to the Zephyr HCP Portal that allows HCPs to:



  1. Send an enrolment lead of an eligible patient

  2. Set Preferences to receive feedback on linked patients including:


    1. to receive feedback directly and/or have feedback provided to your linked clinic nurse (if applicable)

    2. to receive detailed or 6-month summary feedback on linked patients


  3. Set Service Preferences regarding service information about Allied Healthcare Providers to be provided to linked patients and the option to elect for patients to have a home visit from a Zephyr nurse


As part of the Zephyr Program, registered HCPs may receive in accordance with their preferences:



  1. Written or verbal correspondence reporting on how their linked patients are managing on their treatment and any potential health or wellbeing needs that have been identified through their patient’s interactions with the Zephyr team

  2. Feedback on services and information provided to the patient and their parent/carer through the Zephyr program

  3. Optional reminders to contact the patient to arrange due clinical tests and/or appointments such as; blood tests, and/or any other clinical tests or appointments


HCP Registration into the Zephyr Program can occur via online consent to these terms and conditions and privacy statement. The HCP Registration process is initiated by a Biogen team member based on communication with eligible HCPs.


Enrolment into the Zephyr Program (for patients) can occur via calling the Zephyr Program and completing enrolment over the phone, a linked HCP sending an enrolment lead and then calling the patient and completing enrolment over the phone, or by going to the Zephyr patient website www.zephyrprogram.com.au and completing the online enrolment form. Upon receipt of a patient enrolment request, the Zephyr team will verify their eligibility before the enrolment can be accepted.


You may opt out of Zephyr, or components of the program such as emails, at any time by calling 1800 937 497(NZ 0800 897 227) or writing to the Zephyr Program at Partizan, Reply Paid 86597, North Sydney NSW 2059 Australia (no stamp required).


Biogen reserves the right to amend and/or cease funding the Zephyr Program and/or terminate the Zephyr Program without notice at any time.
